Job summary

Adaptive Home Health, LLC is seeking a full-time Physical Therapist to manage an in-home caseload across Texas. You will perform comprehensive assessments, deliver skilled PT visits, and develop plans of care emphasizing gait, transfers, strengthening, and home safety, with occasional weekend coverage as needed.

You will work with a productivity-based pay model at $75 per point (28 points per week) aiming for annual compensation of $100,000–$130,000, with schedule flexibility and field-based

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and thorough documentation practices.
• Willingness to travel daily within assigned territory.
• Ability to notify RN/MD of condition changes and document rationale clearly.

Responsibilities

• Own the caseload with therapy plan, progression, and outcomes.
• Perform in-home assessments and deliver skilled PT visits.
• Develop and execute plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, monitor progress, and adjust as needed.
• Coordinate with care team and manage treatment/outcomes; supervise PTA as appropriate.
• Coach patients and caregivers with practical home programs.
