Job summary

Adaptive Home Health is seeking a licensed Physical Therapist to join our Texas field-based team. You will own caseloads, deliver in-home PT visits, and drive outcomes with a focus on gait, balance, transfers, and home safety.

You’ll work under a productivity-based model ($75 per visit) with a 30-point weekly expectation and schedule flexibility to support patient access and care coordination across service areas.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating condition changes to RN/MD with clear rationale.
• Clear communication and meticulous documentation.
• Reliable follow-through on orders, frequencies, and goals.
• Valid driver's license with daily travel across territory.

Responsibilities

• Own the caseload as a case manager for therapy plans and outcomes.
• Deliver comprehensive in-home PT visits and assessments.
• Develop and execute plans of care targeting gait, transfers, and mobility.
• Set measurable goals and track progress, adjusting as needed.
• Coordinate with care team to align orders and maintain care momentum.
• Supervise PTA where appropriate and document care.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
