Circadia Health is seeking an on-site Nurse Practitioner to join our field team in Walnut Creek, CA. You will deliver direct patient care, act on AI‑driven health signals, and coach facility staff to integrate Circadia into clinical workflows.

This is a full-time role requiring Monday–Friday presence at partner facilities. You will work with clinical ops/product teams to optimize patient outcomes and reduce readmissions.

Qualificações

• CA NP license (ANCC) is required.
• Minimum 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation; company bills on your behalf.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English required.

Responsabilidades

• Direct patient care: conduct initial and follow-up assessments; adjust plans of care.
• Tech → bedside integration: train staff to use Circadia in workflows.
• Data into action: interpret trends and prioritize high‑risk patients.
• Close the loop: complete documentation within the same day.
• High‑volume rounding: perform 30+ visits/day across SNFs when needed.
• Mentor & collaborate: onboard nurses and share best practices with teams.
• Regional flexibility: cover sites within a 30–40‑mile radius; mileage covered per policy.
