Circadia Health is seeking a full-time Nurse Practitioner to lead in-field patient care across Central New Jersey partner facilities. You will interpret AI-generated health signals, adjust care plans, and train facility teams to embed Circadia workflows into daily care.

The role requires 2+ years NP experience, NJ licensure, and willingness to travel locally. Competitive salary with comprehensive benefits and growth opportunities are offered.

Qualifications

• Certified and licensed NP in NJ; ANCC (or equivalent)
• 2+ years NP experience with complex adult populations; independent practice
• Eligible for Medicare participation; company bills on your behalf

Responsibilities

• Direct patient care: initial and follow-up assessments; update plans of care; coordinate with providers.
• Tech - bedside integration: train staff; integrate Circadia into clinical workflows.
• Data to action: interpret insights; prioritize high-risk patients; drive timely interventions.
• Documentation: complete notes within 24 hours of encounters.
• Mentor & collaborate: onboard NPs; share best practices with teams.
• Regional coverage: sites within 30–40 miles; mileage covered per policy.
