Job summary

Qualifications

• 3 years mental health/SUD or community support/case management experience required.
• Experience with severe mental health diagnoses and dual diagnosis.
• Knowledge of community resources is critical.

Responsibilities

• Needs assessment and ISP formulation; crisis stabilization, ADL prompting, coaching and teaching; client feedback.
• Decisions on services and interventions; pursue collateral contacts; adhere to risk management protocols.
• Intake/needs assessment, ISP reviews, progress notes, payeeship forms and discharge paperwork.
• Conforms to ODMH, CARF and third‑party payer standards.
• Interact with peers, supervisors, family, and community partners to advance care quality.
• Participate in meetings and supervision; share knowledge and engage in continuing education.
• Extensive driving to appointments (75% travel) in a company vehicle.
