Job summary

Signature Health, Inc. is seeking a Case Manager in the Finance division to support behavioral health services. The role focuses on developing treatment plans, coordinating transportation, and maintaining client engagement in community settings.

Applicants should have 1–3+ years in case management, at least a high school diploma, and a valid Ohio driver's license. BLS certification and knowledge of community resources are required.

Qualifications

• High School Diploma or equivalent required.
• Bachelor's Degree in related social services field preferred.
• 1–3+ years of case management or relevant experience preferred.
• Valid Ohio driver’s license and proof of insurance required.
• AHA Basic Life Support (BLS) certification required.
• Knowledge of community resources and ASAM levels of care.
• Comfort delivering services in community-based settings and client residences.

Responsibilities

• Diligently develop, prepare and present treatment program work material to clients.
• Plan, coordinate, and provide reliable transportation to clients to access resources.
• Work hours mainly in community-based settings, including client homes.
• Maintain clear communication and professional boundaries with clients.
• Assist in crisis intervention and prevention in the community.
• Participate in quality assurance and utilization review activities.
• Complete productivity and documentation requirements.
• Attend staff meetings and development programs.
