Job summary

The Freedom Center in Mount Vernon, Ohio seeks an entry-level Case Manager to coordinate services for adults and adolescents with behavioral health needs. This full-time, onsite role focuses on intake assessments, referrals, and ensuring continuity of care for each client.

You will collaborate with primary counselors to develop and monitor treatment plans, arrange housing, medical care, transportation, and other supports through community partners.

Qualifications

• High school diploma or equivalent.
• Valid Ohio driver license, two years of driving experience, proof of full auto insurance.
• Ability to pass a BCI check and post-offer/pre-employment drug screen.
• CDCA credential preferred (or obtained within one year of hire).

Responsibilities

• Completing intake-based needs assessments and routine follow-ups.
• Documenting services and required data for client records and funding sources.
• Collaborating with primary counselors to develop, implement, and monitor treatment plans and client progress.
• Coordinating housing, medical care, food, employment, and transportation through community partners.
