Job summary

The Black Mental Health Corporation is seeking a part‑time to full‑time Case Manager in Ohio. You will provide Community Psychiatric Supportive Treatment and coordinate care with a multidisciplinary team to improve stability and quality of life for clients.

Responsibilities include engagement, assessment, care coordination, skill development, progress monitoring, and thorough documentation in line with OhioMHAS standards. Flexibility in schedule is valued.

Qualifications

• Minimum Ohio-based provider qualifications per Ohio Administrative Code 5122-29.
• Bachelor’s degree in behavioral health or related field required or equivalent combination.
• Experience in case management and coordinating services within behavioral health.

Responsibilities

• Engage clients with ongoing outreach and follow-up to support service participation.
• Coordinate services and share information with providers using signed releases.
• Link clients to community resources including housing, transportation, crisis services.
• Provide skill-building interventions for daily living and coping skills.
• Monitor progress and update treatment plans as goals are met or changed.
• Maintain complete clinical records per OhioMHAS standards.
• Participate in staff meetings and required trainings.
• Provide transportation when allowed by policy.
• Ensure confidentiality and compliance with state, federal, and regulatory requirements.

Education

Bachelor’s Degree in a behavioral health field

Associate’s Degree and two years of relevant behavioral health experience

High School Diploma/GED and three years of relevant experience in behavioral health

Bachelor’s Degree in a non-behavioral health field and one year of behavioral health experience
