Job summary

Community Correction Association in Youngstown, OH seeks a Case Manager to perform needs assessments, ISP development, and crisis stabilization for clients with mental health and substance use needs. You will document progress and coordinate services with providers; advocate for resources and ensure compliance with standards.

Travel to appointments (75%) is required. Proficiency with Credible EHR and standard office tools is expected.

Qualifications

3 years mental health/SUD or community support/case management experience.
Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
Knowledge of community resources is critical.
CDCA is required if a candidate does not have a Bachelor’s degree; must obtain prior to hire or within 45 days.
Experience using computers, EHR (Credible), office phone and cell phone, email, fax/copy machine.

Responsibilities

Needs assessment and formulation of ISP; crisis stabilization and coaching.
Decisions on additional services and treatment interventions as needed.
Intake/needs assessment, ISP and reviews, progress notes, discharge paperwork.
Conforms to agency standards and third-party payer obligations.
Engage with peers, supervisors, and service providers to support client progress.
Participate in agency meetings, supervision, and ongoing education.
Travel to appointments (up to 75%) using a company vehicle.
