Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager - BH to oversee community-based behavioral health care for dually eligible enrollees. You will lead an interdisciplinary team, coordinate with providers, and ensure seamless transitions and comprehensive care planning.

The role requires a Master’s in social work or a licensed mental health clinician, with experience in care coordination and Medicaid/Medicare programs. Travel within the Commonwealth is expected.

Qualifications

• Master's degree in social work or mental health counseling and independent license required.
• BH Clinician capable of independently serving people with complex needs.
• Prior care coordination/case management experience preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage enrollees in homes and community settings, addressing cultural and linguistic needs.
• Coordinate with providers and community resources for seamless care.
• Conduct Comprehensive Assessments and functional assessments (MDS-HC or successor).
• Lead the interdisciplinary care team to develop holistic care plans.
• Oversee utilization of long-term services to keep enrollees in their chosen location.
• Assist access to housing, transportation, food, and social services.
• Educate enrollees about benefits under Medicare and Medicaid.
• Promote self-management and preventive care, close gaps in care.
• Follow up after hospitalizations to ensure continuity of care.
