Job summary

The Harris Center for Mental Health and IDD is hiring a Care Coordinator to provide case management and care coordination for individuals in a housing program affiliated with the Coalition for the Homeless. You will transport consumers, conduct assessments, and collaborate with a multidisciplinary team led by an LPHA.

Requirements include a 4-year degree in a social/human services field and at least one year in the mental health field.

Qualifications

• Bachelor's degree from a four-year accredited college or university in a social, behavioral, or human service field is required.
• Minimum of one year experience in the mental health field is preferred.
• Must maintain a valid Texas Driver's License and be insurable under Agency policy.
• The ideal candidate will be required to transport consumers using their personal vehicle and will be reimbursed for mileage.

Responsibilities

• Conduct comprehensive clinical assessments to determine service needs and track progress toward treatment goals.
• Collaborate with the treatment team to develop personalized recovery plans and obtain LPHA approval for consumers on caseload.
• Ensure services meet eligibility criteria and comply with guidelines.
• Deliver skills training and psychosocial rehabilitation to address severe mental illness.
• Provide crisis interventions and assist consumers in accessing necessary care and services.
• Provide education on mental illness, medication importance, and Harris Center curriculum.
• Monitor progress toward goals and ensure compliance with eligibility requirements.
• Meet productivity standards and demonstrate ethical conduct.
