Job summary

The Harris Center for Mental Health and IDD is seeking a Service Coordinator to empower individuals with intellectual disabilities by guiding access to medical, psychiatric, residential and financial supports. You’ll bridge consumers, representatives and providers to create person-directed care plans that reflect each client’s goals and needs.

Responsibilities include assessments, ongoing welfare monitoring, and maintaining accurate records while complying with safety standards and mandatory

Qualifications

• Bachelor’s degree in a social, behavioral, or human service field or related qualifies candidates.
• Strong written and verbal communication skills.
• Ability to work autonomously in a fast-paced environment.

Responsibilities

• Conduct comprehensive assessments following policy and procedures.
• Monitor client welfare quarterly and tailor to individual needs.
• Maintain and update consumer files with data integrity.
• Document timely and accurately across records.
• Travel to consumer homes and engage with diverse individuals.
