Job summary

The Harris Center for Mental Health and IDD in Houston seeks a Care Coordinator to provide community-based case management for participants in a housing program. You will work within a multidisciplinary team to develop recovery plans and ensure eligibility and regulatory compliance, with transportation support for consumers.

A Bachelor's in a social/behavioral/human service field and at least one year in mental health are required. A Texas driver’s license is essential for consumer transport.

Qualifications

• Bachelor’s degree in a social, behavioral, or human service field is required.
• Must have strong communication and interpersonal skills.
• Ability to work collaboratively in a multidisciplinary team and manage time effectively.

Responsibilities

• Conduct comprehensive clinical assessments to determine service needs and progress.
• Develop personalized recovery plans with LPHA validation for your caseload.
• Ensure services meet eligibility criteria and regulatory guidelines.
• Deliver skills training and psychosocial rehab to address mental illness.
• Provide crisis interventions to stabilize symptoms.
• Assist consumers in accessing necessary care and services.
• Educate on mental illness and medications per Harris Center curriculum.
• Monitor progress and ensure policy compliance.
• Maintain ethical conduct and documentation per regulations.
