Qualifications

• High School Diploma/GED or higher with 5+ years of ABD/SSI clinical experience.
• Associate's degree with 3+ years of clinical experience.
• Unrestricted LPN/LVN license in TX with 3+ years of clinical experience.
• CHW certification with 3+ years of clinical experience.
• MS Office proficiency (Word, Excel, Outlook).

Responsibilities

• Complete face-to-face assessments in members' homes to address unmet needs.
• Travel up to 75% of the time, including travel up to 100 miles from the assigned work location, to meet business needs.
• Meet documentation and turnaround times based on state contractual requirements.
• Time management and organizational skills.
• Coordinating members' services.
• Ability to create, edit, save and send documents, spreadsheets and emails.
• Reside within commutable distance of assigned duties.
• Works on Service Plan.
