Qualifikationen

2+ years of experience in a community health or healthcare setting.
Proficiency with Microsoft Office applications.
Reliable transportation and ability to travel within assigned territory.

Aufgaben

Assess, plan and implement care strategies individualized for each patient and directed toward appropriate level of care.
Identify and initiate referrals for social service programs (financial, psychosocial, community, state).
Manage the care plan throughout the continuum of care as single point of contact.
Communicate required health-related information to stakeholders to ensure quality coordinated care.
Advocate for patients and families to ensure needs and choices are represented.
