Job summary

UnitedHealthcare is seeking a Field Based HSS Clinical Coordinator RN in Mission, TX. You will assess, plan, and implement person-centered strategies, connect members with resources, and coordinate care across the continuum with providers and families.

Travel up to 75% within Mission and nearby areas; remote work is allowed. Requires 2+ years in a community health setting, MS Office proficiency, reliable internet, and a valid US driver's license. Bilingual in English and Spanish is a plus.

Qualifications

• 2+ years of experience in a community health or healthcare setting
• Intermediate level of proficiency with Microsoft Office applications
• High-speed internet at residence
• Access to reliable transportation and valid US driver’s license with the ability to travel within assigned territory

Responsibilities

• Assess, plan, and implement person-centered strategies
• Connect members with financial, psychosocial, community, and state resources
• Coordinate care across the continuum and serve as a key point of contact
• Advocate for members and families to ensure their needs and preferences are heard and supported
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes
• Travel up to 75% of the time within the Mission, TX area and surrounding areas
