Job summary

The Whole Child is seeking a Temp Shallow Subsidy Case Manager in Downey, CA. This part-time role supports families, children, and individuals to achieve housing stability through light-touch case management within a Community Based Scattered Site Model.

The position involves 80% driving, field visits to homes and community settings, and collaboration with landlords and service providers to coordinate resources and housing plans. Temporary through June 30, 2027, exempt conditions apply.

Qualifications

• Bachelor’s degree in social work, psychology, or other mental health related discipline.
• Minimum of 4 years of work experience related to Housing Navigation or rehabilitative services in a mental health setting.

Responsibilities

• Conducts comprehensive needs assessments with families, TAY, and individuals to obtain needed resources.
• Practices Trauma Informed, Housing First, Harm Reduction, and Low Barrier approaches.
• Completes and submits progress notes within 72 hours in HMIS.
• Provides light touch case management: independent living, money management, employment linkage, benefits establishment.
• Develops housing plans with clients addressing short-term and long-term goals.
• Identifies resources to address homelessness, domestic violence, and substance use issues.
• Provides ongoing case management to monitor progress and adjust plans as needed.
• Maintains communication with landlords to promote housing success.
• Provides tenant education on rights, discrimination, and landlord communication.
• Attends meetings to present housing services and eligibility criteria.
• Follows up for six months post-placement to ensure permanency.
• Conducts monthly follow-up meetings in office and in the field.
• Takes a proactive approach to retain housing and independent living.
