Job summary

PsychPros, Inc. is seeking a Housing Case Manager in Altadena, CA to provide intensive case management for clients experiencing homelessness in a temporary assignment.

You will conduct intake, enroll clients, link to temporary housing, assess needs, and develop service plans. Transporting clients to essential appointments is included as needed.

Qualifications

• 2 years of experience in the human services field OR a bachelor’s degree in Social Work or in a related field.
• Bachelor’s degree in Social Work or related field preferred.

Responsibilities

• Provide intensive case management to clients in a permanent supportive housing program with chronic health conditions.
• Conduct intake and enrollment with eligible clients.
• Assist clients with accessing temporary housing until permanent housing is secured.
• Support clients with the lease-up process and develop individualized service plans.
