Job summary

The Salvation Army USA Western Territory is seeking an Interim Subsidy Case Manager to provide transitional case management and coordinate with landlords to ensure successful tenancies in the Shallow Subsidy Program.

You will work with a multidisciplinary team to assess client needs, create stability plans, and monitor progress, while maintaining HMIS data and participating in trainings on tenant rights in California.

Qualifications

• Bachelor's degree preferred, but not required.
• At least 2 years of experience with affordable housing services.
• At least 1 year of experience with homeless people who have mental illness and/or substance use issues.
• Excellent communication skills.
• A valid California Class C Driver License.
• A criminal background check is required.
• Driving within LA County is required.
• Driving test and MVR check required.

Responsibilities

• Participate in weekly staff meetings and CIS meetings as needed.
• Conduct initial assessment to determine client's specific needs.
• Create a Housing stability plan with the client, including documentation of financial independence.
• Coordinate related services and provide ongoing monitoring of housing stability.
• Track and document participant progress through case management.
• Collaborate with care team and health professionals to maintain stable permanent housing.
• Assess participants' wellness, needs, and abilities to connect them with resources.
• Establish and maintain relationships with landlords and property managers.
• Conduct annual habitability inspections of permanent housing units.
• Certify eligibility quarterly and annually for continuing households (in person).
• Submit check requests for property management/landlords monthly.
• Provide tenant education on rights, responsibilities, and discrimination.
• Attend mandatory annual training on tenant/housing rights in California.
• Maintain communication with landlords to address questions or concerns.
• Document services provided in HMIS (GIRP).
• Attend LAHSA-approved trainings monthly as assigned.
• Provide crisis intervention resources as needed.
• Maintain a caseload of up to 35 clients.
• Respond to grievances from landlords and participants with resolutions per policy.
• Adopt a Housing First, Harm Reduction, Conflict Resolution, and Trauma‑Informed Care approach.
• Maintain tracking systems for participant and program information.
• Execute duties per the Shallow Subsidy SRS guidelines.
