Job summary

Brilliant Corners seeks a Temporary Case Manager in Los Angeles to deliver intensive case management for 20-30 participants, aligning with a Housing First approach. You will assess needs, create individualized plans, and connect clients to housing, health, and community resources while collaborating with housing, property, and employment teams.

The role emphasizes in-home, community, and office meetings, with travel throughout Los Angeles County (approximately 75% field time).

Qualifications

• Direct mental health/intensive case management experience preferred.
• Experience with homelessness, mental illness, chronic health issues, or substance use favorable.
• Valid California driver's license and access to a personal insured vehicle.

Responsibilities

• Conduct assessments and develop individual service plans to support housing stability.
• Assist with housing placement, applications, leases, and ongoing retention.
• Coordinate with partners and document in Salesforce, CHAMP and BOX.
• Provide crisis intervention and connect clients to health and community resources.
