Job summary

Pacific Health Group is hiring a field-based Housing Navigator to support Medi-Cal members in Mendocino County communities, including Modesto, Turlock, Ceres, Riverbank, and Oakdale. You will travel locally, perform assessments, develop care plans, and coordinate housing resources in a hybrid arrangement.

You will work in a fast-paced, community-based environment focusing on housing stability, collaboration with providers, and real-time documentation under Medi-Cal guidelines.

Qualifications

• Strong understanding of housing system and local resources required.
• 3–5 years experience in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.

Responsibilities

• Provide housing stability and navigation services in the community.
• Coordinate housing tenancy, deposits, and ongoing case management.
• Engage with housing authorities, shelters, landlords, and partners.
