Job summary

SAMMinistries in San Antonio, TX seeks a Rapid Re-Housing Case Manager to provide field-based, direct client services to families or individuals experiencing homelessness. You will perform comprehensive case management, housing navigation and retention in a fast-paced environment.

The role requires a Bachelor’s degree in Social Work or related field, and at least two years of case management experience with HMIS; bilingual Spanish is preferred.

Qualifications

• Bachelor’s Degree in Social Work or related field with housing and case management knowledge.
• Two years of case management experience; HMIS experience preferred.
• Experience with homeless services or vulnerable populations is a plus.
• Ability to coordinate projects independently and within a team.

Responsibilities

• Provide field-based case management focused on housing stabilization, budgeting and financial literacy.
• Guide clients through housing search, applications and lease signing.
• Develop individualized service plans addressing short-term and long-term goals.
• Utilize Trauma-Informed Care, Housing First, Motivational Interviewing and related practices.
• Maintain documentation and ensure compliance with program contracts and HIPAA.
