SAMMinistries in San Antonio, TX is seeking a Youth and Young Adult Housing Case Manager to provide field-based, direct client services for clients in the Joint Component Transitional Housing – Rapid Re-housing program. The role focuses on stabilization, identifying needs, and linking education, health, life skills, and housing resources.

You will work toward permanent housing by offering case management, housing searches, landlord collaboration, and documentation.

Qualifications

• Bachelor’s Degree in Social Work or related field with housing and case management knowledge.
• Experience in homeless services, mental health, or with vulnerable populations.
• Experience working with young adults, LGBTQ and diverse populations preferred.
• Flexible schedule including some evenings and weekends.

Responsabilités

• Provide field-based, client-centered case management for housing stabilization.
• Assist with housing searches, applications, leases, move-in, and stabilization.
• Develop individualized service plans addressing short- and long-term goals.
• Utilize Trauma-Informed Care, Housing First, Motivational Interviewing and related practices.
• Maintain documentation compliant with program and HIPAA requirements.
• Coordinate with landlords, services, and benefits to support housing stability.
