Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead patient-centered hospice care by conducting comprehensive assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with focus on pain and symptom management
• Serve as case manager coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort
• Provide education and emotional support to patients and families
• Help families navigate complex decisions with empathy and clarity
• Maintain accurate, timely documentation and participate in care planning meetings
• Support quality and compliance standards
