Job summary

VitalCaring – Deliver Care That Brings Comfort and Dignity is seeking an On Call Hospice Registered Nurse (RN) to lead patient-centered hospice care in field-based settings. You will manage symptoms, develop comfort-focused plans, and collaborate with a multidisciplinary team to honor patients’ goals with compassion.

The role requires an active RN license, at least 1 year of clinical nursing experience, and strong communication in sensitive situations.

Qualifications

• Active RN license is required.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care and develop individualized care plans.
• Coordinate care across the interdisciplinary team.
• Educate, support, and guide patients and families throughout the journey.
• Maintain accurate documentation and participate in care planning.
