VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered hospice care in field-based settings, prioritizing comfort and dignity. You will oversee plans of care, manage symptoms, and advocate for patients and families with a compassionate, clinical approach.

The role emphasizes autonomy within an interdisciplinary team, ensuring high-quality, personalized care in homes and facilities as part of end-of-life services.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care, developing and managing comfort-focused care plans.
• Serve as the care coordinator, collaborating with physicians, social workers, chaplains, and aides.
• Provide education, emotional support, and guidance to patients and families throughout the journey.
• Maintain accurate, timely documentation and participate in care planning and IDT meetings.
