Job summary

Transitions LifeCare seeks an experienced Registered Nurse Case Manager to lead the hospice care team in the field, guiding care plans and coordinating with physicians, social workers, and aides.

You’ll travel to patients’ homes and facilities across our service area, delivering compassionate, evidence-based nursing care and education, while acting as a trusted advocate for patients and families.

Qualifications

• Current RN license in North Carolina and active CPR certification.
• Minimum two years of recent clinical nursing experience.
• Ability to travel throughout the service area and provide home-based care.

Responsibilities

• Lead and coordinate the interdisciplinary care team for assigned patients.
• Develop, implement, and manage individualized hospice care plans.
• Deliver skilled nursing assessments, education, and symptom management.
• Ensure care aligns with physician orders, regulations, and hospice best practices.
• Provide education to patients and families on disease progression and care options.
• Complete timely, accurate clinical documentation and comply with workflows.
