Job summary

Brigade Health is seeking an experienced Hospice RN Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. This role emphasizes leadership, case management, and regulatory adherence during extensive home-based visits.

The position requires travel across multiple locations, 1-10 visits daily, and on-call leadership to ensure 24/7 patient care coverage. Proficiency with EMR and Microsoft Office is expected.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Minimum of 3 years of clinical RN experience, with hospice experience required.
• At least 1 year of supervisory or management experience in a hospice setting.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards. JCAHO knowledge preferred.
• Valid RN license in the state of practice.

Responsibilities

• Lead with Compassion & Excellence in Hospice Care and oversee patient care.
• Supervise, mentor, and ensure competency of the hospice care team.
• Oversee care plans, evaluations, treatments, and patient outcomes.
• Monitor care to ensure compliance with Medicare and hospice regulatory standards.
• Review and approve care plans in the EMR system with adherence to Medicare Conditions of Participation.
• Participate in agency quality assurance initiatives and drive performance improvements.
• Lead interdisciplinary team meetings with clinicians and the medical director.
• Provide education, guidance, and documentation support for hospice team members.
