Job summary

Transitions LifeCare is seeking a Weekend Float RN Case Manager to deliver expert hospice nursing care in patients’ homes and community settings. This role emphasizes autonomy, leadership, and collaborative care across teams while maintaining the highest standards of comfort and dignity for patients and families.

The RN will conduct scheduled visits, manage admissions, and step into the Case Manager role as census requires.

Qualifications

• Current RN license in North Carolina is required.
• Minimum two years of recent clinical nursing experience.
• Current CPR certification is required.
• Must be able to travel throughout the service area and provide care in homes and facilities.

Responsibilities

• Conduct scheduled hospice visits throughout the community.
• Assess patient needs and provide skilled nursing care per the hospice plan of care.
• Monitor patient conditions and implement interventions to maximize comfort and quality of life.
• Provide clinical leadership and coordinate care across disciplines.
