Job summary

Transitions LifeCare seeks a registered nurse to serve as a Community-Based Hospice RN Case Manager. The role involves independent field visits to patients in homes and care settings across our North Carolina service area, with daily travel and autonomous clinical decision-making.

You will lead the care team, advocate for patients and families, and develop personalized care plans that reflect goals and comfort. A strong clinical background in hospice is essential.

Qualifications

• Current RN license in North Carolina.
• Minimum two years of clinical nursing experience.
• Current CPR certification.
• Reliability and transportation to travel across service area.

Responsibilities

• Lead patient-centered hospice care by developing and managing individualized care plans.
• Coordinate the interdisciplinary care team for assigned patients.
• Deliver nursing assessments, interventions, education, and symptom management.
• Monitor patient conditions and adjust plans using clinical judgment.
• Ensure care aligns with physician orders, regulations, and hospice standards.
• Build trusting relationships with patients and caregivers.
• Provide education on disease progression, medications, and hospice services.
• Support families in complex healthcare decisions with compassion.
