Job summary

Harbors Home Health & Hospice is seeking a compassionate Registered Nurse Case Manager to deliver skilled, patient-centered end-of-life care in the field. You will coordinate plans of care, perform comprehensive assessments, and manage symptoms while supporting patients and families.

The ideal candidate works independently, maintains strong clinical and communication skills, and collaborates with an interdisciplinary hospice team to ensure dignified, respectful care.

Qualifications

• Current Washington State RN license is required.
• Valid driver’s license and reliable transportation.
• Hospice or home health experience preferred.

Responsibilities

• Manage an assigned caseload of hospice patients.
• Complete comprehensive nursing assessments and ongoing reassessments.
• Develop and coordinate individualized plans of care.
• Provide symptom management and medication oversight.
• Educate and support patients, families, and caregivers.
• Communicate and collaborate with physicians and the interdisciplinary team.
• Complete timely and accurate clinical documentation.
• Participate in on-call rotation as required.
