Job summary

The Care Team Hospice is seeking a full-time Registered Nurse Case Manager to deliver skilled nursing and coordinated end-of-life care to patients in their homes and facility settings, collaborating closely with the interdisciplinary team. Standard hours apply with on-call rotation optional for extra pay.

Qualifications

• Active RN license required
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and the interdisciplinary team
• Educate patients and families on symptom management and end-of-life care
• Document assessments, interventions, and care plans in the EMR HCHB
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation
