Job summary

Laphcc is seeking an experienced Home Health RN Case Manager to cover Fort Worth, TX and the surrounding Tarrant County area. This field-based role focuses on independent caseload management with skilled nursing visits and comprehensive care planning in patients' homes.

Responsibilities include accurate documentation (OASIS and visit notes), ongoing assessments, and collaboration with physicians and the clinical team to ensure high-quality, compliant home health care.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management.
• Ability to work independently while maintaining communication with the clinical team.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the team.
• Complete accurate, timely clinical documentation including OASIS and visit notes.
• Monitor patient progress and communicate updates to the care team.
• Educate patients and caregivers on disease management, meds, safety, and care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
