Job summary

RNInk is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. You will conduct skilled nursing visits, assess patients, and coordinate plans of care with the care team in the home setting.

Ideal candidates hold a Texas RN license, have recent home health case management experience, and can document thoroughly in EMR systems while traveling locally as needed.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong assessment and documentation skills.
• Ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates to the care team.
• Provide patient and caregiver education related to disease management, medications, safety, and goals of care.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Territory: Tarrant County area - Hurst, Euless, Bedford, Grapevine, Irving

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
