Care Advantage, Inc. is seeking a Home Care Sales Specialist for Roanoke, VA. This 100% field-based role covers Roanoke and surrounding markets, taking full ownership of your territory to develop strong referral networks and position Care Advantage as the provider of choice.

You will collaborate with a high-performing sales and operations team focused on accountability, results, and shared success, building relationships with hospitals, SNFs, and senior living communities.

Qualifications

• Proven track record in B2B healthcare sales.
• Previous experience managing Veterans referrals (preferred).
• Backgrounds in Home Care, Home Health, Hospice, or Assisted Living/Senior Living.

Responsibilities

• Develop and nurture referral relationships with hospitals, SNFs, assisted living communities, and rehabilitation centers.
• Call on social workers, case managers, discharge planners, and facility leadership.
• Present Care Advantage's services as patient-centered care solutions.
• Communicate our clinical capabilities, responsiveness, and service quality.
• Build and execute weekly territory and account plans.
• Schedule and lead face-to-face meetings and in-services.
• Partner with operations and clinical teams to ensure seamless care transitions.
• Drive market share growth and strengthen brand presence.
• Utilize CRM tools and company resources to achieve results.
