Job summary

Potomac-Community-Services seeks a LEAD Case Manager to collaborate with the Washington County Sheriff’s Department, Health Department, and Mobile Crisis to provide harm reduction services, peer support, and community case management. The role focuses on diverting potential law enforcement intervention and promoting engagement with community resources.

The position requires a Bachelor’s degree in Human Services (preferred) and 2+ years in behavioral health with adults, including experience with

Qualifications

Bachelor’s degree in Human Services preferred.
Minimum 2 years experience in behavioral health working with adults.
Experience coordinating and delivering services to clients with homelessness, substance abuse, and/or mental health issues.
Strong interpersonal skills.
Excellent verbal and written communication skills.

Responsibilities

Learn PCMS Core Values and demonstrate their application in all areas of work.
Serve as liaison for PCMS with community agencies and local mental health providers.
Attend and participate in monthly staff meetings and caseload reviews.
Maintain confidentiality of client and staff information in line with HIPAA and state/federal laws.
Record time and manage calendar accurately using the approved systems.
