The role requires a bachelor’s degree (or 2+ years of case management) and a valid California driver’s license. The candidate will collaborate with a multidisciplinary team and community partners within a harm-reduction framework.

Qualifications

• Bachelor’s degree required or equivalent experience.
• Experience working with culturally diverse populations.
• Valid CA driver’s license and vehicle.
• Must not be on any type of legal supervision.

Responsibilities

• Provide intensive wraparound services on site and in the field.
• Engage clients with rapport and motivational interviewing; meet clients where they are.
• Coordinate care with a multidisciplinary team and external partners.
• Maintain case notes and billing records; participate in data collection.
