CVS Health is hiring a Community Health Worker to support West Virginia Medicaid members through outreach, care coordination, and resource navigation. This field teleworker role requires 70% travel and residency in WV, with home visits, telephonic outreach, and collaboration with the care team.

The CHW will document interactions in the electronic health record, address social determinants of health, and help members access services, transportation and eligible programs while maintaining HIPAA

Qualifikationen

• High School Diploma /G.E.D. or higher.
• 2+ years relevant experience in healthcare, community-based, social services or case management.
• Complete an approved CHW certification program within 12 months of hire.
• Proficiency with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• Reliable transportation and valid driver's license with ability to travel up to 70%.

Aufgaben

• Conduct home visits to assist with needs, locate resources, schedule appointments and educate on services, medications and conditions.
• Complete telephonic and/or email outreach to members, agencies and care providers.
• Collaborate with care team members and community partners to address gaps in care.
• Assess gaps in care or social determinants of health and formulate interventions.
• Document all outreach and care coordination in electronic health records.
