CVS Health’s Aetna Better Health of West Virginia is seeking a Community Health Worker (CHW) to join a care management team. This full-time, field-based role requires WV residency and travel to member homes and community sites.

You will assess needs, coordinate services, and advocate for members within Medicaid programs. The position emphasizes outreach, education, and collaboration with care teams, while maintaining HIPAA compliance and documenting care activities in the EHR system.

Qualifications

• High School Diploma or GED required.
• Complete an approved CHW certification program within 12 months of hire.
• Proficiency with PCs, keyboarding and multiple software systems; MS Office applications.
• Must have reliable transportation and a valid driver's license; travel up to 70% of the time.

Responsibilities

• Conduct home visits to assist with needs and connect to community resources.
• Telephonic and email outreach to members, agencies, and providers.
• Collaborate with care team and community partners.
• Assess gaps in care and develop interventions to address needs.
• Document outreach and care coordination in electronic health records.
• Maintain PHI and adhere to HIPAA guidelines.
• Travel extensively to member homes and community locations as needed.
