CVS Health's ABHWV program seeks a Community Health Worker to join a field telework team serving West Virginia residents. The role requires WV residency with extensive travel (70%+), visiting member homes, assessing needs and coordinating care. You will educate members on services and navigate community resources.

As part of a strong care management team, you will work to bridge members with providers, document interactions in the EHR, and support families in navigating Medicaid services.

Qualifikationen

• WV residency required for field telework.
• Willingness to travel 70% of the time in Northern WV regions.
• Ability to educate on services, medications and conditions.
• Maintain member confidentiality and PHI per HIPAA.

• Conduct home visits to assist members with resources and services.
• Complete telephonic and/or email outreach regarding member needs.
• Collaborate with care team and community partners.
• Assess gaps in care and coordinate interventions.
• Document outreach and care coordination in EHR system.
• Travel extensively to member homes and community locations.
