Job summary

VARIETY CARE INC. is seeking a Community Health Worker (CHW) to serve as a frontline, relationship-based member of the Population Health team in Oklahoma City. The CHW will improve access, health literacy, engagement, and outcomes by acting as a liaison between patients, families, the care team, and the community.

The CHW collaborates with clinical teams to reduce avoidable ED visits and close gaps in care while supporting PCMH goals. Strong communication and cultural mediation are essential.

Qualifications

• High school diploma required or GED.
• Minimum 2 years of patient-facing healthcare/community support experience.
• C3 CHW core competency training preferred within 12 months if Medicaid billing is applicable.
• Valid driver’s license required.
• Willingness to travel 25–30% within the Oklahoma City metro area and 5–10% to rural sites.
• Bilingual English/Spanish preferred.

Responsibilities

• Outreach and case-finding to connect individuals and groups to Variety Care services.
• Guide patients through scheduling, referrals and follow-up care.
• Provide basic screenings and assist with social needs like housing/food access.
