Variety Care Inc. is seeking a Community Health Worker (CHW) to serve as a frontline liaison between patients, care teams, and the community in Oklahoma City. The CHW will improve health literacy, engagement, and access while supporting value-based care and PCMH goals.

The role emphasizes relationship-based care, home visits, outreach, and linkage to community resources. A high school diploma or GED and strong communication skills are required; bilingual abilities are a plus.

Qualifications

• High school diploma or GED required or equivalent.
• Minimum two years in patient-facing healthcare or community support.
• Experience with C3 core competencies preferred within 12 months.

Responsibilities

• Outreach and case-finding to connect individuals to Variety Care services.
• Builds and maintains resource inventory of community assets and partners.
• Guides patients through scheduling, referrals, and follow-up care.
• Identifies barriers to care across clinical, social, financial, transportation, and digital access.
• Tracks referrals and ensures timely closure; escalates unresolved barriers.
• Coordinates CHW activities with clinical staff to support unified care plans.
• Provides basic screenings and connects patients to essential resources.
• Engages in health education and patient activation activities.
