Job summary

Variety Care is seeking a Community Health Worker (CHW) to serve as a frontline, relationship-based member of the Population Health team. The CHW will work closely with care teams to improve access, health literacy, engagement, and outcomes across the patient experience.

The role follows CHW core competencies and collaborates with clinical staff to reduce avoidable ED visits, close gaps in care, and support value-based care goals.

Qualifications

• High school diploma or GED.
• Minimum of two years in a patient-facing healthcare or community support role.
• C3 core CHW training preferred at hire; required within 12 months if Medicaid billing is planned.
• Travel willingness: approximately 25–30% in Oklahoma City metro and 5–10% to rural sites.

Responsibilities

• Conduct outreach and case-finding to connect individuals, families, and groups to Variety Care services.
• Provide patient navigation through scheduling, referrals, follow-up, and connection to resources.
• Offer basic screenings and connect patients to essential resources (food, housing, etc.).
• Engage communities to improve health literacy, engagement, and activation within Population Health.
