Variety Care Inc is seeking a Community Health Worker (CHW) in Yukon, OK. The CHW will serve as a frontline liaison between patients, families, the care team, and the community to improve access, health literacy, engagement, and outcomes across Variety Care’s patient experience.

In collaboration with clinical teams, the CHW will conduct outreach, provide basic screenings, support care coordination, and help patients navigate healthcare and social services with an emphasis on culturally

Qualifications

• High school diploma or GED.
• Minimum of two years in a patient-facing healthcare/community support role.
• C3 CHW core competency training preferred; if Medicaid billing is involved, must complete within 12 months of hire.
• Ability to travel 25–30% in OKC metro and 5–10% to rural sites.
• Valid driver’s license is required.

Responsabilités

• Outreach & case-finding to connect individuals and groups to Variety Care services, including home visits and community events.
• Navigate care processes: scheduling, referrals, follow-ups, and linking to internal/external resources.
• Provide basic screenings and connect patients to essential resources (food, housing, telehealth support).
• Maintain longitudinal engagement with assigned high-risk patients and support patient activation.
• Act as cultural mediator; deliver culturally and linguistically appropriate health education.
• Collaborate with clinical teams and community partners to support a unified care plan.
• Document activities in the EHR and participate in quality improvement activities.
• Adhere to HIPAA, CHW ethics, and Variety Care policies; participate in required training.
