Job summary

Variety Care is seeking a Community Health Worker (CHW) in Oklahoma City to serve as a frontline member of the Population Health team, improving access, health literacy, engagement and outcomes across the patient experience.

The CHW will work closely with clinical teams to reduce avoidable ED visits, close gaps in care, and support value-based care goals; requires two years in patient-facing roles and CHW training.

Qualifications

• High school diploma or GED required.
• Minimum of two years in a patient-facing healthcare or community-based role.
• C3 core competency CHW training preferred within 12 months if Medicaid billing.
• Ability to travel within Oklahoma City metro area and to rural sites.

Responsibilities

• Outreach & Case-Finding: Proactive outreach to identify eligible individuals and connect them to Variety Care services.
• Patient Navigation & Care Coordination: Guide patients through scheduling, referrals, follow-up care, and resources.
• Direct Services & Basic Support: Conduct basic screenings and connect patients to essential resources.
• Longitudinal Engagement & Patient Activation: Maintain ongoing engagement and support patient self-management.
• Cultural Mediation & Health Education: Provide culturally and linguistically appropriate health education.
• Advocacy & Capacity Building: Act as patient advocate and build community capacity.
