Job summary

The Community Health Worker (CHW) at Variety Care Inc. serves as a frontline, relationship-based member of the Population Health team, connecting patients, families, the care team, and the community.

They work to improve access, health literacy, engagement, and outcomes across Variety Care's patient experience, collaborating with clinical teams to reduce avoidable ED visits and support value-based care goals.

Qualifications

• High school diploma or GED required.
• Minimum of two years in a patient-facing healthcare or community role.
• C3 CHW core competency training preferred; required within 12 months if Medicaid billing is intended.
• Ability to travel ~25–30% in OK and 5–10% to rural sites.
• Must have a valid driver's license.

Responsibilities

• Conduct proactive outreach and connect individuals and groups to Variety Care services.
• Guide patients through scheduling, referrals, and follow-up care.
• Provide basic screenings and connect patients to food, housing, and other resources.
• Engage in health education and community outreach to improve health literacy.
• Maintain documentation in the EHR and coordinate care with clinical teams.
