Job summary

Variety Care in Oklahoma seeks a Community Health Worker (CHW) to be a frontline, relationship-based team member focused on improving access, health literacy, and outcomes for patients. The CHW will liaison among patients, care teams, and the community, supporting value-based care and PCMH goals through proactive outreach, education, and care coordination.

Qualifications

• High school diploma or GED required.
• Minimum of two years in a patient-facing healthcare or community support role.
• Willingness to travel 25–30% in OKC metro and 5–10% to rural sites.
• Able to work independently within structured workflows and priorities.
• Lived experience aligning with the community served preferred.

Responsibilities

• Outreach and case-finding to connect individuals and groups to Variety Care services, including through home visits and community events.
• Build and maintain a current resource inventory of community assets and referral partners.
• Prepare and disseminate outreach and health education materials.
• Guide patients through scheduling, referrals, and follow-up care, connecting to internal and external resources.
• Identify barriers to care across the patient journey and work to resolve them within workflows.
• Track referrals and ensure timely closure; escalate unresolved barriers as needed.
• Support post-ED and post-hospital transitions with timely follow-up and continuity of care.
• Coordinate CHW activities with clinical staff to support unified care plans.
• Provide basic screenings and supportive services within scope, and connect patients to essential resources.
• Promote telehealth use and help patients access virtual visits when appropriate.
• Maintain engagement with a defined panel of high-risk patients and support activation of care plans.
• Educate patients and families with culturally and linguistically appropriate health information.
• Advocate for patient needs and resources, building community capacity through outreach.
• Participate in documentation, data collection, and quality improvement activities.
