Job summary

Reema Health is hiring a Community Health Navigator (Community Guide) to build relationships with members, guide them toward greater engagement in their own healthcare, and address social barriers using Reema's technology platform.

Responsibilities include outreach, caseload management (80-120 members), coordinating referrals, and documenting care plans while maintaining confidentiality. Roles may evolve with company growth.

Qualifications

• Must be solutions-oriented and able to improvise paths forward.
• Strong verbal and written communication skills.
• Proficient with technology and mobile/desktop tools.
• Self-motivated to stay productive on a remote caseload.
• Collaborative and able to build team connections in a remote setting.

Responsibilities

• Conduct phone and in-person outreach to enroll members.
• Manage a caseload and support team members during peaks.
• Assess needs and build member-centered care plans.
• Educate and empower members to engage with health activities.
