Job summary

Reema Health is seeking a Community Health Navigator to actively engage members and guide them toward better health outcomes. You will enroll members, manage a caseload, assess needs, and develop member-centered care plans using Reema’s technology platform.

Expect collaboration with a multidisciplinary team in a startup environment that values flexibility and growth. You will build relationships with community resources, coordinate referrals, and document progress while maintaining HIPAA

Qualifications

• Self-motivated to stay productive on a remote caseload.
• Collaborative in a remote team with occasional in-person meetings.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema’s care model.
• Build and manage a caseload (80-120 members) and cover others during absences.
• Assess member needs to build a member-centered care plan.
• Establish trusting relationships and follow through in a timely manner.
• Coordinate referrals and schedule appointments.
• Document all member work thoroughly in the Reema App and maintain confidentiality.
