Job summary

Reema Health in Louisville, KY is seeking a Community Health Navigator to build trusted relationships with members and guide them toward greater engagement in their own healthcare. You’ll manage a caseload, conduct outreach, assess social determinants of health, coordinate referrals, and document all work in the Reema App while adhering to HIPAA.

The role requires 18+, Monday–Friday core hours, field-based travel (25–50%), and a compassionate, solutions-oriented approach.

Qualifications

• 18 years of age or older.
• Available to work Monday–Friday during core business hours.
• [If field-based:] Valid driver's license, insurance, and reliable transportation
• Offers contingent upon passing a pre-employment drug screening and background check
• Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA
• Use technology-based tools and systems daily

Responsibilities

• Conduct outreach and enroll members in Reema's care model
• Build and manage a caseload (80-120 members) and cover for others as needed
• Assess member needs, including social determinants of health, to build a member-centered care plan
• Establish trusting relationships and follow through in a timely manner
• Explain the importance of health activities and help overcome hesitations
• Coordinate referrals and schedule appointments
• Hit monthly goals and daily metrics through independent workload management
• Engage with members via phone, text, email, and in-person visits
• Connect members to community and health resources, including transport when needed
• Maintain professional boundaries and confidentiality; consult the multidisciplinary team
• Document all member work in Reema App and keep care plans current
• Comply with HIPAA and patient privacy requirements
